We are actively accepting patients at this time, however, due to high patient volume we do have a waiting list. Please contact the office to place your name on the list.
We encourage patients to schedule appointments in advance and arrive on time with insurance cards, drivers license, and any medical information that may be helpful for your visit.
For new patients, please arrive 15 minutes early to complete any paperwork.
Same day appointments are available on first com first serve basis. It is best to call first thing in the morning to try and obtain one of these appointments.
To ensure quality care, we do not treat patients over the phone. You will need to schedule an appointment for evaluation and any needed testing. However, we do offer telehealth appointments.
We do not treat patients we have not seen such as calling in prescriptions or offering medical advice prior to initial visit.
As a courtesy, we do provide appointment reminders. It is still the patients responsibilities to make arrangements for an appointment that was previously made.
In order to be respectful to the medical needs of our patients, please be courteous and call to cancel your appointment if you are unable to attend so that it can be reallocated to someone in need of treatment. We ask that you call as soon as possible to cancel your appointment.
No Show Policy:
For new patients: the first no show will result in a notification letter. If a new patient does not present for two appointments in a row, we will no longer be able to accept you as a patient.
For current patients: the first no show will result in a notification letter. A second no show will result in a notification letter and reminder of this policy. After the third missed appointment, you will receive a letter notifying you of your termination from the practice. This letter will also be sent to your insurance company. You will have 30 days from the letter date where our providers will only be able to treat you on an emergency basis.
We offer a self-pay discount for individuals without medical coverage.
Payment is expected at the time of service.
We will bill your claims to your insurance for any service that can be billed under you medical coverage. We will not be able to submit claims to you prescription coverage for services such as vaccines and or immunizations. Depending on your insurance company this service may only be covered under you prescription coverage and will need to be provided to your pharmacy. This is typical for any Medicare patients and you are advised to call your insurance carrier to see if this covered. If you or our staff are aware that a service may not be covered, you will be asked to sign an Advanced Beneficiary notice stating you were informed ahead of time and that you may need to pay out of pocket for the service.
